不同抗生素方案在盆腔炎性疾病治疗中的成本—效果分析

摘要：目的 评价不同抗生素方案治疗盆腔炎性疾病（PID）的经济学价值，通过成本-效果分析为临床合理选择治疗方案提供循证依据。方法 选取2023年6月至2024年12月期间本院收治的120例PID患者作为研究对象，采用随机数字表法分为A 组（40 例）、B 组（40 例）和C 组（40 例）。A 组接受静脉注射头孢曲松钠+ 多西环素+ 奥硝唑方案；B 组接受静脉注射左氧氟沙星+ 奥硝唑方案；C 组接受静脉注射莫西沙星单药方案。比较三组的临床疗效、不良反应发生率，并运用成本- 效果分析法评估其经济学效益。结果 三组临床总有效率比较，差异无统计学意义（P>0.05）。A组与B、C组间不良反应发生率比较，P<0.05，差异有统计学意义，A组不良反应发生率最低。成本-效果分析显示，A 组的成本-效果比（C/E）最低，增量成本- 效果比（ICER）分析表明，相对于B 组和C 组，A 组每增加一个单位效果所需追加的成本最低，具有更优的经济学价值。结论 在PID的治疗中，头孢曲松钠+多西环素+奥硝唑联合方案在保证相近临床疗效的同时，兼具更低的不良反应发生率和最优的成本- 效果比，是更具经济学优势的临床选择。

Cost-Effectiveness Analysis of Different Antibiotic Regimens in the Treatment of Pelvic Inflammatory Disease

Abstract：Objective To evaluate the economic value of different antibiotic regimens in the treatment of pelvic inflammatory disease (PID) and provide evidence-based support for rational clinical selection through cost-effectiveness analysis.Methods A total of 120 PID patients admitted to our hospital between June 2023 and December 2024 were selected as study subjects and randomly divided into Group A (40 cases), Group B (40 cases), and Group C (40 cases) using a random number table. Group A received intravenous ceftriaxone sodium + doxycycline + ornidazole; Group B received intravenous levofloxacin + ornidazole; Group C received intravenous moxifloxacin monotherapy. The clinical efficacy, incidence of adverse reactions, and economic benefits via cost-effectiveness analysis were compared among the three groups.Results There was no statistically significant difference in the total clinical effective rate among the three groups (P>0.05). The incidence of adverse reactions in Group A was significantly lower than in Groups B and C (P<0.05). Cost-effectiveness analysis showed that Group A had the lowest cost-effectiveness ratio (C/E). Incremental cost-effectiveness ratio (ICER) analysis indicated that, compared to Groups B and C, Group A required the lowest additional cost per unit of effect gained, demonstrating superior economic value.Conclusion In the treatment of PID, the ceftriaxone sodium + doxycycline + ornidazole combination regimen provides comparable clinical efficacy while offering a lower incidence of adverse reactions and the optimal cost-effectiveness ratio, making it a more economically advantageous clinical choice.
